Long-Term Care Insurance: Eldercare Solution

When I suddenly had to become a fulltime caregiver to my elderly parents, both with health problems and starting to develop dementia (namely Alzheimer’s), I had never even heard of Long-Term Care Insurance. After we burned through their life savings, and then started chipping away at mine, I was advised to apply for financial assistance for them through the government’s Medicaid system–a program for those at the poverty level. It was quite a long process with mounds of paperwork and numerous investigations, but finally my parents were approved.

I was so happy that monetary help would finally be on the way, until I discovered that the financial assistance would only pay to put my parents in a nursing home, not even in Assisted Living, and with very little help to keep them in their own home.

Since their levels of care were so different (my mother needed most things done for her), there weren’t any facilities that would allow them to be together. They’d be across the street from each other in different wings of the home. After fifty-five years of marriage, my parents were adamant about wanting to be together in their own home, in their own bed, where they could continue to cuddle and kiss–as they so frequently did. And, since my father was so “difficult” with a terrible temper and quite a long record of manipulative disruptive behaviors, the homes didn’t want to deal with him anyway.

It was challenging, but I committed to keeping my parents in their own home and attending Adult Day Health Care five days a week. Then, with the help of two marvelous caregivers, after four more years of loving each other–they passed, just a few months apart. Even though caring for every aspect of my parents’ last years was the hardest thing I have ever done–I am proud to say I gave them the best end-of-life I possibly could.

Had I only known to insist that we buy Long-Term Care Insurance for them prior to their illnesses–their years of in-home care could have been paid for, and I could have saved myself so much heartache, not to mention a small fortune. I encourage you to learn from my mistake and look into LTC insurance long before you need it–for your loved ones as well as yourself. Like fire insurance, hopefully, you’ll never have to use it.

Also, call your local Area Agency on Aging, or Department of Aging, and ask if there are any financial programs, waivers or grants available in your area that you can apply for.

STARTLING STATISTICS

· An estimated 4.5 to 5 million Americans have Alzheimer’s disease. In a Gallup poll, 1 in 10 Americans said that they had a family member with Alzheimer’s, and 1 in 3 knew someone with the disease.

· Increasing age is the greatest risk factor for Alzheimer’s. One in 10 individuals over 65 and nearly half over 85 are affected. Rare, inherited forms of Alzheimer’s can even strike individuals in their 30’s and 40’s.

· A person with Alzheimer’s disease will live an average of eight years and as many as 20 years or more from the first onset of symptoms.

· More than 7 out of 10 people with Alzheimer’s disease live at home, where family and friends provide 80 percent of their care. The estimated value of this informal care is $257 billion annually.

· One half of the U.S. population has a chronic condition. More than one quarter (26.6%) of the adult population provide care for a chronically ill, disabled or aged family member or friend, which translates to more than 50 million people.

· 37% of caregivers are living in the same household as the person they care for. 54% are between 35 and 64 years of age. 59% of the adult population either is or expects to be a family caregiver, and 2 million more caregivers will be needed in the next twenty years.

· An estimated 43% of Americans age 65 or older will spend time in a nursing home. By 2012, 75% of Americans over age 65 will require long-term care. Long-term care costs are rising at 6% annually.

· The annual cost of Alzheimer’s care in the U.S. is at least $100 billion, and will soar to at least $375 billion by mid-century, overwhelming our health care system and bankrupting Medicare and Medicaid.

· Alzheimer’s disease costs American business $61 billion a year, which is equivalent to the net profits of the top 10 Fortune 500 companies. $24.6 billion covers Alzheimer health care, and $36.5 billion covers costs related to caregivers of individuals with Alzheimer’s, including lost productivity, absenteeism and worker replacement.

THREE WAYS TO PAY FOR LONG-TERM CARE

1. Pay for in-home caregivers and assisted living/nursing homes out of pocket. This is expensive and can often deplete a family’s life savings.

2. Meet a very specific poverty level and qualify for government assistance through the Medicaid program. Unfortunately, options are limited, only paying for nursing homes that accept Medicaid.

3. Buy a Comprehensive Long-Term Care Insurance policy. This protects your family’s assets from the rising costs of caring for someone who needs full time care. An employer might pay the tax-deductible premiums. Consider buying it at a younger age, when more affordable and accessible. It must be bought before a major illness strikes. Medicare and regular health insurance does not pay for long-term care. The average cost for a person who needs long-term care is $40-$70,000 annually, depending on where you live, plus the cost to the family caregiver who may have to leave their job.

QUESTIONS TO ASK YOUR INSURANCE AGENT

–Is the coverage comprehensive, meaning it includes all levels of care: in the home, assisted living, board & care, and nursing/dementia facilities?

–What is the daily benefit?

–Is there 5% annually compounded inflation protection?

–What is the elimination period?

–Is it a lifetime benefit period or a limited time benefit policy?

–Is there a spousal discount?

–Can you hire caregivers privately as well as from an agency?

–Is the home care benefit based on a daily, weekly or monthly maximum, and if the benefit is not used, can it be used in the future?

–Does it cover home care coordination of services?

–How many ADL’s (Activities of Daily Living) does it take to trigger a claim?

–Is there a time limit for filing a claim?

–Does it cover the cost of Adult Day Care & Adult Day Health Care, hospice and respite programs?

–Is it a tax-qualified plan?

–Is the company highly rated and have they ever raised premiums?

–Can you see the company’s published annual audit to check their track record for paying claims?

Toll Free numbers have gone a long way; nowadays there are hundreds of toll free companies and resellers out there that provide the same basic features. Enhancements on telephone applications enabled toll free service to acquire most if not, all of the telephone applications inherent in landline services.

Toll Free numbers as explained before are call forwarding service, which effectively makes it 2 phone calls. The first call from the original caller to the toll free service provider, then another call from the toll free service provider to the service subscriber. This is a great opportunity to charge for more while legally advertising a cheaper price. This annoys consumers but found it difficult to change service provider or acquiring a new number. Vanity numbers are hard to find and takes a week to register. For most businesses the cost of changing service providers is very expensive on wasted time and opportunities

A lot of consumers don’t know that their numbers are “portable” and can be used on any service provider assuming proper legal procedures have been done; this normally takes just as long as registering a new vanity number. There are people who also thought that Telco’s such as AT&T have more numbers in their database for them to choose from, what they don’t realize is that all toll free companies get their number availability from the same place.

Today even experienced toll free service subscriber still fall for tricks made by service provider to make money. The best advice for a good deal is the same as any other consumer advice, shop around, be thorough, and find great toll free service providers out there.

Life As A Juggling Woman

When we moved 6000 miles away from our families 22 years ago, I certainly never imagined I would be working in the business my grandfather established way back when. Way back when … there was no internet, no low cost international telephone service, no e-mail, and no digital photography. My children are grown up now, and theoretically I COULD leave the house and find outside employment, but I have now chosen not to take that path. This time the decision is a calculated one.

Throughout my family’s childhood and teen years, I solved the “where to be first issue” by working from home. My hard-earned M.Sc. degree in Human Resource Administration was shelved – although I would like to think I applied some of the key principles to running our in-house human resources. As a fluent English speaker in a foreign country, armed with the latest computer equipment in my own home when computers were fairly new even in offices, I opened an English-language word processing business out of a corner of my living room.

My clients came from the nearby academic centers and new hi-tech industry park. As word processing became more sophisticated, I moved on to desktop publishing and was soon creating books, brochures, and journals. I attended seminars, read the literature and soon expanded my services to offer copywriting and marketing communication. Over the years my portfolio grew and I felt
a special frisson whenever I saw a company with my marketing material succeed.

All the while, the children were growing up, and although often pressured from the deadlines and demands of not one boss, but many — as is the plight of the independent business person — I was able to “be there” for them and participate in school and club events.

Over the years, I co-authored a book, established, published and wrote an online magazine with two women partners, and with them
also built an online business. All this while, my children graduated high school, served in the army, traveled abroad, returned, left home, returned, had a baby, worked abroad, returned, got a girlfriend (who knows? he doesn’t tell me anything…), and we built a house. Now I have a fabulous corner office looking out on the garden and my husband has his own
sanctuary upstairs.

And then my father surprised me during a routine touch-base
telephone call, which he later backed up with an e-mail note. “I’ve been thinking… Maybe you see a way to use the internet for our business? Is there a way you could direct something
like that?”

Well, blow me away. I just happened to be at a crossroads. My husband was preparing to set out on a two-week long male-bonding trek in the Himalayas, I was recuperating from a torn miniscus operation, my son was nearing the end of his army duty, the downturn in high tech and in tourism had negatively effected my bottom line, I cherished drop in visits to my little granddaughter, and I needed an opportunity I could sink my teeth into.

When Joel headed east to trek, I headed west to create a new interface to a 90-year-old family business, Maurice Goldman Fine Jewelry.

Over the last eight months, the learning curve has been steep.
Within 2 weeks of opening our eBay store, the fraudsters were running rampant.

David Bloom wrote in from Cremona, Italy, with ready cash for a $20,000 sapphire ring, and a strong recommendation that we use an escrow service to protect him from losing his hard earned cash. It’s true that he never spoke about protecting us from losing our hard earned merchandise… At the eleventh hour, well, actually at 8 AM in my pajamas in front of the computer screen, with the aid of my calm and analytical husband, I avoided our first theft in the virtual world. We learned that not all escrow sites are created equal, and that the one our “customer” “recommended” was a fraud. In his last e-mail note to me, Mr. Bloom lamented that the site was phony, and that he had just suffered a loss of $20,000. Couldn’t we have told him sooner?(!)

Other would-be sales included stolen credit cards (this is apparent when the buyer suggests that you take MORE money than you the posted sales price to cover charges), more fraudulent escrow sites, money transfer deals, and a bank check swindle. As Joel points out, the crooks are always a step ahead.

My work vocabulary has grown exponentially, as has my respect for the business world in general, and my father in particular.
Our business issues are the same: to source new products, to market and sell to a growing customer base, and to avoid theft and fraud. But whereas my Dad deals with the real world, my
business is virtual. I find the global reach of the virtual world tremendously satisfying and very neat. Customers tell us that our online presence means they can acquire goods otherwise unavailable in their small towns. So here I am in Israel, promoting and selling jewelry that is in New York, to customers around the world, without leaving the house. The process of building and handling the internet extension of our family
business, and combining family, home and business
brings me full circle. My cup runneth over.

Bitterness Can Make You Sick

I watched as the razor-sharp knife of the skilled surgeon performed the delicate, life-saving task of repairing an aneurism in the elderly man’s stomach. I had a close-up view from a stool beside the operating table.

At that time several years ago, I shared an office building with this surgeon and his partners. He had finally said “yes” to my request to “let me watch one day.” I knew it would be interesting, but I didn’t know that I’d be blessed with a very graphic life lesson in that operating room. I’ll spare you most of the very gory details. I had only been used to “verbal gore,” not red stuff. Thank God there are those who are called to be surgeons–I’m not!

Anyway, back to the Operating Room. The task was to cut out the aneurism and replace it with a synthetic graft.The doctor had located the aneurism, had clamped off the arteries, and had begun to remove the crud (medical term, I’m sure) from the affected section of artery. I was amazed at the amount of stuff (another medical term) that he piled into the container. This HAD to be life threatening. The blood is the life flow of the body. If it can’t flow where it’s needed, the person can’t live. At that moment, dressed in my surgery garb and totally grossed out, a powerful insight flashed into my spirit. This blockage is a lot like bitterness. Something negative happens in your life, and little by little, its poison accumulates, blocking the flow of life energy. Before my eyes was a living metaphor for BITTERNESS. Think through it with me.

CUMULATIVE CHOICES CREATE CUMULATIVE HARM.

That patient had made lifestyle choices that resulted in an accumulation of unhealthy material in the critical arteries that sustained his life.

The bitter person also makes choices–dwelling on the offense, mentally (or physically) performing little acts of revenge, refusing to forgive. The poison accumulates.

THE PROBLEM GROWS, AND ITS IMPACT SPREADS.

From a physical standpoint, the blood flow is critical. Block its flow to the heart or other vital organs and you have a catastrophic impact.

Each person has a similar flow of emotional and spiritual energy. The consequences of a blockage in these life-giving channels are just as tragic as an aneurism can be. The problem grows and its impact spreads.

Imagine that you released a drop of red food coloring into a beaker of water. At first you’d see a red dot. Soon you’d see pink water. Like the drop of food coloring, anger that is nurtured grows and deepens. It begins to affect every “system”–relationships, attitudes, motivation, and productivity.

DRAMATIC INTERVENTION IS NECESSARY TO SAVE A LIFE.

Thankfully, under the care of this experienced and competent surgeon, the patient got a chance to live an estimated 20 to 30 years longer. He would be freer to do the things that he enjoyed. Yes, there would be a painful period of recovery from this major surgery, but it was worth it.

You may or may not need professional help to release yourself from the bitterness you’ve harbored. Notice I said release YOURSELF, not release the other person(s). They are going on their merry way. (Of course, this can make you madder.)

I’ve had clients tell me, “If I forgive, that person will go scott-free.” News flash! The other person(s) are already out there ppursuing their life interests. It’s YOU who is suffering for holding onto bitterness. It you whose irritability and short fuse is robbing you of peace in relationships you value. It’s you whose physical health is deteriorating because of anger. It’s you whose sleep is disturbed and whose positive energy is sapped. It’s you whose creativity is stifled and whose productivity is diminished.

The Girl Next Door

When you think about the girl next door, usually it brings pleasant thoughts. She is a lady we have seen around, exchanged greetings and smiles – walked away feeling good. The part about feeling good has a lot to do with the calm atmosphere and the casual meeting. There was no crowd, no expectations of trying to be picked up, and you were not concerned about being rejected. Instead, it was you and a lady exchanging some small talk with no pressure or competition. Over time as you get to know each other the conversation may get a little more personal and you may decide to get together for dinner or drinks. It seems to come natural.

In a club, the atmosphere is completely different. There is competition and much more intensity in the air. The women don’t know who you are and if they seen you around, they can only judge you by your actions – which may be effected by alcohol. When you do walk up to one of these ladies in a club, right away you are inviting her to go out, have a drink, etc., without her really knowing who you are. No wonder this type of atmosphere is ripe with rejection. Granted a lady may get to know you in a club over time, but there always seems to be another guy around the corner trying to take your place. Probably this competition is the reason for the hurried invite for a personal connection. Sometimes it works, sometimes it doesn’t. But I bet, must guys have had more then their share of rejections in a club and really don’t care for the feeling.

Meeting a girl online is similar to the girl next door concept. She may not be next door, but the atmosphere of little pressure, casual conversation and getting to know each other by online exchange (email, online chatting, etc.), make it a more pleasant feeling. In time, this exchange may blossom into more then friendship. If sparks connect, you will find yourself in love with the girl next door though she may be miles even oceans away.

A lady in a bar who doesn’t know who you are, may say no for that reason alone. She may get to know you in time, but more then likely you have asked her out before she got the chance to know who you are. After a guy has been rejected, more then likely he won’t return to the scene of the accident. You may tell yourself you will let the lady get to know you before making any advances, which is not a bad idea, but by the time she gets to know you, three other guys have asked her out. The girl next door concept in a bar could work, but the competition and the odds are stacked against you.

With the girl next door concept on the internet, you get to know each other before asking intimate questions. And when the time comes to invite the lady into a romantic encounter, she knows who you are and rejection is less likely. Granted, there are jerks on the net whose first email may say “baby, I want to make love to you all night,” but that line seems out of place for a first email. Yet that same line may not seem so out of place the first time you meet a lady in a club. See the difference? I doubt if you would say “baby, I want to make love to you all night” the first time you meet your new lady neighbor. Yet in a club, after a few drinks, you will say just that and it is almost expected.

You can see why the girl next door concept works well with online dating. Ladies get to know the guys, rejection is less, and finding love is more likely. I can’t imagine a better way to get to know the girl next door then from the comfort of your home and computer. The girl next door is as close as your fingers on the keyboard – you just got to go out and find her. With so many online dating services, finding the girl next door should be as easy as ringing her door bell.
